MyWorkChoice offers you and your family dental, vision, life, and medical insurance for as little as $20 a week through Essential Staff Care (ESC).

During open enrollment, MyWorkChoice employees can add, drop, or change their benefit plan by completing this form. Once submitted, you will receive an email confirmation from ESC. Employees can decline benefits plan at any time by reaching out to Essential Staff Care.

If you are currently enrolled and do not wish to make any changes, no action is required.
